Health Care for the Homeless

Any adult or child experiencing homelessness is eligible for this program. This includes anyone who has been homeless within the past year or that is living:

  • Outside or on the street
  • In a shelter or transitional housing
  • With friends or relatives

Health Care for the Homeless serves clients at a variety of shelters and drop-in centers in Minneapolis.

Services and appointments

Services

We provide a variety of health and wellness services to people experiencing homelessness.

Medical care

  • Medication refills
  • Sick visits
  • Minor injuries
  • Pregnancy tests
  • Birth-control
  • Blood pressure checks
  • Check-ups for adults and kids
  • COVID testing and vaccines
  • Flu and vaccine updates
  • Foot care
  • Wound care, skin problems or infections
  • Hepatitis C testing and treatment
  • HIV and STI screening
  • PrEP
  • Tuberculosis (TB) testing

Mental health

  • Therapy
  • Medication
  • Sleep problems
  • Diagnostic assessment
  • Community mental health support

Substance use disorder and harm reduction services

  • Suboxone
  • Clean needles
  • Narcan
  • Help scheduling a comprehensive assessment (Rule 25)
  • Help getting into treatment

Other

  • Care coordination
  • Needs assessment
  • Referral to community services and resources

Medical and mental health visits will be prioritized over visits for paperwork completion. Please visit your primary care provider for forms or paperwork needs.

Appointments

You are not required to have an appointment. Clinic hours vary by location. We recommend you call to confirm a clinic is open.

Endeavors respite program

Respite is a short-term service for patients experiencing homelessness who are too sick to be on the streets or shelter, but not sick enough to be in the hospital. Respite provides a space for patients to recover from an acute health concern or exacerbations of chronic health conditions.

While in respite and before discharge, our team will work with the patient to develop a plan. The plan addresses what is important to the patient and steps towards their recovery.

Respite clients have access to:

  • Private rooms
  • 3 meals a day
  • Shared bathrooms
  • Shared laundry

Eligibility

Anyone experiencing homelessness and willing to take part is eligible. They must also be:

  • Too sick to be on the streets or in a shelter
  • Over 18 years old
  • Having a serious health concern or increase in severity of a chronic health condition
  • Independent in al ADLS (mobility of greater than 100 ft to bathroom/cafeteria)
  • In control of or able to manage bowel and bladder
  • Not intoxicated or not likely to experience withdrawal symptoms
  • Willing to participate in respite program
