How to file Prepare application Complete a certificate of filing for marriage officiant form (PDF, 1MB). Print but do not sign the application. Get your signature on the application notarized (e-notarization is acceptable). Get a readable photocopy of your credentials of license or ordination; or a statement of marriage authority from a spiritual authority. (Do not send originals. Submitted credentials will not be returned.) Gather your application, copies of documents and a $30 check or money order made out to Hennepin County Treasurer.
