SHAPE has been done every four years since 1998. Local public health, human services, and nonprofit agencies use what they learn from the survey to identify and fund programs that improve community health.
Agencies have used information from past surveys to:
- Encourage people to eat more fresh fruits and vegetables
- Reduce tobacco use by children and adults
- Expand access to healthy food
- Increase opportunities for physical activity
