SHAPE surveys

The SHAPE survey is administered every four years by Hennepin County Public Health. The survey helps us understand the most vital health needs of our increasingly diverse community.

The SHAPE 2022 survey had two main components.  One component was a mail survey to a randomly selected set of households in the county.  The second component was a set of survey responses collected in collaboration with a set of community partners located mostly in the Minneapolis area.  These respondents were people who lived by or were served by those community partners.

In both cases, the respondents were asked about their health, diet, exercise, neighborhood, and ability to get health care, and the impact of COVID-19.

About the surveys

How we use the survey results

SHAPE helps Hennepin County Public Health understand differences in health and identify health disparities. These results then inform programs initiated by the county and by community organizations that aim to improve community health and achieve health equity for all.

Local public health, human services, and nonprofit agencies have used information from past surveys to:

  • Encourage people to eat more fresh fruits and vegetables
  • Reduce tobacco use by children and adults
  • Expand access to healthy food
  • Increase opportunities for physical activity

Your participation is important

Participation in this project provides residents a unique opportunity to directly influence health policy and programs in Hennepin County. All responses are anonymous and confidential. Because only some residents are selected to participate, participation in the survey and responses are very important.

The SHAPE public use data files are resources for public health researchers to do analyses with the SHAPE data, going beyond what is presented in the published reports, briefings, and fact sheets.

To access any of the SHAPE public use data files, complete the form below and submit it as directed.

You will get a reply within two weeks. If your use request is accepted, you will be asked to sign a data use agreement form before the data are released to you.
