Child and Teen Checkups (C&TC) Program

Child and Teen Checkups services are for young people who receive healthcare benefits through Medical Assistance, including independent teens and young adults up to age 21, as well as young parents.

Whether you speak English, Spanish, Somali, Hmong, or another language, we will speak with you in your native language. Talk with a Child and Teen Checkups specialist for assistance or information by calling 612-348-5131.

Learn about our program

We will help to connect you with a clinic that provides specialized health checks for our clients. Once you get there, you can expect:

  • A complete physical exam
  • Hearing and vision checks
  • Needed lab tests and immunizations
  • Checks on development and growth
  • Referral to a dentist
  • Information about good physical and mental health, behavior and development

You can use this health check for to meet requirements for Head Start, WIC, school, sports or day care.

Schedule an exam

Children and teens should get health checks at these milestones:

  • Between birth and 1 month
  • At 2, 4, 6, 9, 12, 15, 18, 24, 30, and 36 months
  • Every year after age 3 through age 20

You can make your appointment for a C&TC health check at any one of a variety of clinics in your community, which you can find by using our interactive map. Participating dental clinics

You can make your appointment for a dental checkup at a variety of dental clinics in your community, which you can find by using our interactive map. Child and Teen Checkups Program referral form

This is a secure, online referral form for agencies who work with C&TC for referrals from clients.

Referral forms should be submitted by the sub-contracted agency within 24 hours.
