SHAPE surveys

In 2018, the county will ask 40,000 randomly chosen adults to take a voluntary health survey called SHAPE. The survey is anonymous (we don’t ask for anyone’s name) and responses are combined with those of many other residents taking the survey.

SHAPE includes questions about:

  • Physical and mental health
  • Diet
  • Exercise
  • Lifestyle
  • Ability to get health care

The survey takes about 15 minutes to complete. Households will get a postcard telling them they’ve been randomly chosen to take the survey. That’s followed by a paper survey they fill out and mail back to the county.

Use of survey results

SHAPE has been done every four years since 1998. Local public health, human services, and nonprofit agencies use what they learn from the survey to identify and fund programs that improve community health.

Agencies have used information from past surveys to:

  • Encourage people to eat more fresh fruits and vegetables
  • Reduce tobacco use by children and adults
  • Expand access to healthy food
  • Increase opportunities for physical activity

Results of the 2018 survey will be made public in early 2019.

The SHAPE public use data files are resources for public health researchers to do analyses with the SHAPE data, going beyond what is presented in the published reports, briefings, and fact sheets.
