Long-Term Care Infection Prevention Program

We support long-term care facilities’ efforts to prevent resident infections through education, resource development, and relationship building. We provide free, non-regulatory consultations to help facilities in Hennepin County identify infection prevention program strengths and improvement opportunities. We also provide tailored, evidence-based resources for facility staff to apply to their infection prevention and antibiotic stewardship program development.

Coalition

Infection prevention and antibiotic stewardship are shared responsibilities and require an ecosystem of partnerships and a commitment to continuously improve. The Long-Term Care Infection Prevention Coalition of Hennepin County promotes a culture of infection prevention and antibiotic stewardship excellence by providing professional development opportunities and a network for facilities and community partners to share best practices.

Antibiotic and urine culture stewardship training

Long-term care health professionals – including clinicians, infection preventionists, nurses, pharmacists, and quality improvement teams – are invited to use this training series to build their capacity to establish and maintain robust antibiotic stewardship programs. Multidisciplinary subject matter experts apply stewardship best practice recommendations and improvement strategies by presenting case studies, demonstrating the use of new and existing templates and answering frequently asked questions.

Hand hygiene training kit

The Basics: Hand Hygiene in the Long-Term Care Setting

This training is intended to provide basic hand hygiene education for long-term care frontline staff in an interactive format including demonstrations, activities, and discussion. It is designed to be facilitated by the facility’s infection prevention leader(s). Upon completion of this training, participants should be able to:

  1. Identify the important moments for hand hygiene in the long-term care setting.
  2. Differentiate when alcohol-based hand sanitizer is preferred and when soap and water is preferred for hand hygiene.
  3. Demonstrate proper hand hygiene technique.
  4. Identify strategies to promote a positive hand hygiene culture.

Infection preventionist toolkit

This toolkit incorporates continuous improvement principles intended to help new infection preventionists (IPs) develop data-driven risk assessments that inform infection prevention and control (IPC) program activities. The content within the text will guide you on how to identify IPC areas of strength, areas where opportunities for improvement exist, and areas where data are missing.

Transmission-based precautions

Transmission-based precautions training kit

This training is intended to provide basic transmission-based precautions education for long-term care frontline staff in an interactive format including demonstrations and activities. It is designed to be facilitated by the facility’s infection prevention leader(s). Upon completion of this training, participants should be able to:

  1. Demonstrate proper technique for donning and doffing personal protective equipment (PPE)
  2. Describe the different types of transmission-based precautions.
  3. Determine what PPE is appropriate based on the type of transmission-based precautions in place.
  4. Apply transmission-based precautions knowledge to long-term care situations.
