Youth Homelessness Demonstration Program

The Youth Homelessness Demonstration Program (YHDP) is an initiative of the United States Department of Housing and Urban Development (HUD). Hennepin County is a YHDP community. Learn more about HUD's YHDP initiative here (hudexchange.info).

We need your input! If you work with youth experiencing housing instability or homelessness in Hennepin County:

  1. Review the Coordinated Community Plan – Progress Report to identify goals and objectives that you or your program may have impacted or influenced.
  2. If you have an update for any Action Steps, submit a survey.

Coordinated Community Plan

In 2022, to become a YHDP community, we developed the Hennepin County Coordinated Community Plan to Prevent and End Youth Homelessness (PDF). The plan was developed by the YHDP Core Team, which included members from the Youth Action Board (YAB), non-profit organizations, Hennepin County, and other technical assistance partners. The plan represents a continued commitment and focus to make homelessness rare, brief, and non-reoccurring for youth and young adults (YYA).

Tracking progress

In 2024, the Youth Action Board worked with Hennepin County to develop the Coordinated Community Plan – Progress Report. The Progress Report is meant to be a living document. It includes status updates for all goals, objectives, and action steps outlined in the Coordinated Community Plan. This will guide our community’s ongoing and future work to prevent and end youth homelessness.

Youth Homelessness Demonstration Program funding

The following projects were selected through a facilitated community process involving the YHDP Core Team. YHDP funding is coordinated by Hennepin County’s Continuum of Care.

YHDP Projects:

  • Connections to Independence (C2i) - Foster Youth to Independence (FYI) Voucher Supportive Services
  • Oasis for Youth - Foster Youth to Independence (FYI) Voucher Supportive Services
  • Hennepin County - Youth and Young Adult Navigation Services
  • Institute for Community Alliances (ICA) - HMIS support and training to youth projects
  • The Link - Permanent Supportive Housing (PSH)
  • The Link - Rapid Rehousing (RRH)
Alternative funding for youth homeless services

Hennepin County administers Youth Homeless and Housing Services (YHHS) funding. This funding is for youth-specific homeless prevention, street outreach, drop-in centers, emergency shelters, and supportive housing. Funding was distributed for use from April 1, 2024 to December 31, 2026.
