I wish to participate voluntarily in the Find your 5 Challenge for the purpose of healthier dietary behavior.
WHEREAS, the City Department of Parks and Recreation has established a Find your 5 fruits and vegetables program for residents; and WHEREAS this program is being set up at no cost to the participants; THEREFORE, in consideration of my participation in this program at no cost to me, I hereby release the city, the county, its agents, and its employees from any and all liability for injury or property damage that occurs while participating in this program. I also agree that the Hennepin County Human Services and Public Health Department, or its designee, has the right to publish the names of Find your 5 Challenge winners in print and electronic communications including news items, promotional materials, and on the Web.
