Birth certificate or record application You can get a birth certificate only if you have tangible interest. Some birth records are confidential and can only be obtained by certain people. Uncertified copies of birth records are also available, but are for information only and are not accepted for legal purposes. Before you purchase an uncertified copy, make sure an it is acceptable for your needs. Certified copies can generally be issued at time of application. Copies applied for at the University of Minnesota location will be mailed. How to start There are two ways you can begin your application process. Option 1: Complete (except for signatures) the birth record application (PDF) and print it. Submit it in person/walk-in, by appointment, by mail or by fax. Option 2: Start your application, pay online, print your application and submit it by fax. There is an additional $7 service fee charged by VitalCheck. Pay by American Express®, Discover®, MasterCard® or Visa®. Cost There are several payment options options available. Fees are non-refundable, as per MS 144.226. First certified copy: $26 Each additional certified copy of the same record: $19 Uncertified copy: $13 Submit application Review your options for submitting your application. Additional information Due to limited data availability, sometimes uncertified copies can't be processed while you wait and will have to be mailed to you. If you are making changes to the birth record with a parent notice (within 45 days of birth) we are required to hold your application for two business days. Use the amendment process for requesting changes if it has been more than one year since the date of birth.
