South Minneapolis service co-location

Partners are working together to combine services and find new ways of providing service at the South Minneapolis Human Service Center.

Services and partners will move into the building in phases, starting in October 2021.

As onsite services become available, they will be offered by appointment.

Apply online or by phone for cash, food, child care and health care assistance and other human services programs. You don't need to come to our offices to get our help.

Adult probation

Probation staff work closely with clients, community organizations, victims, and criminal justice partners to break down barriers and increase client and community growth. South Minneapolis focus areas include neighborhood probation and domestic abuse units.

Hennepin Healthcare East Lake Clinic

The clinic provides community-centered primary and specialty care, including family medicine, women’s health, diagnostic imaging, lab services, care coordination, and more. It also offers prenatal, post-natal and pediatric care.

Mental Health Center

The Mental Health Center serves adults, children, adolescents, and families who live in or receive services from the county. It provides diagnostic assessments, psychiatric evaluations, individual and group therapy, case management and vocational supports.

Office of Multicultural Services

Staff help connect limited English proficiency residents to county and partner services in their preferred language. They also work with service providers throughout the community and build relationships with people the county serves. Walk-in services will not be available until 2022. 

Human services

Human services staff connect residents to services that help with basic needs and improve safety, stability, self-sufficiency and well-being. The new model is based on providing information and increased options for access, including on-site appointments, community access points, and phone and internet access that will be available onsite at the human service center. For best results, residents should connect with staff before visiting a site because in many cases they can access everything they need from wherever they are.

WIC: Women, Infants and Children

WIC is a nutrition and breastfeeding program that serves pregnant women, new mothers, babies, and young children. Nutrition education and counseling are the cornerstones of this public health program. WIC also offers breastfeeding support and referrals to health and social services.

Tenants continuing to operate in the space

Briva Health is MNsure’s largest enrollment partner in Minnesota. MNsure-certified assisters are available to Minnesotans and their families to enroll in health coverage.

Seward Pharmacy is a neighborhood pharmacy providing prescriptions services, emergency first-aid services, medical counseling, and health care products.
