TeenHOPE, Pathways, and Family Home Visiting changes
On January 1, 2023, Hennepin County will take on administration of the TeenHOPE, Pathways and Family Home Visiting programs. We're excited to offer these services to eligible families with young children.
Hennepin County will administer these programs for eligible county residents, including families who live in the City of Minneapolis.
Families living in Bloomington, Edina and Richfield can contact Bloomington Public Health for services at 952-563-8900.
Program participants will continue to receive familiar, high-quality services, with minimal disruption. Our goal is for the change to feel seamless to the people served by the programs.

TeenHOPE and Pathways
These programs will become part of the Well-being area of Human Services. They provide support and services for teen parents. Services include:
- Help working toward a high school diploma or GED
- Family home visiting that includes parenting support and education
- Family planning support and education
- Connections to help with post-secondary options or career exploration.

Family Home Visiting
This program will become part of the Public Health Family Health area.
Family Home Visiting (FHV) promotes positive birth outcomes and healthy child development. It also provides family-centered and community-based public health nursing services.
Home visitors work with families in their homes and other community settings. They also partner with health care providers and local community agencies. They do this to help to support healthy outcomes and positive parent-child relationships. Healthy growth and development depend on this.
