Homeless to Housing program

Hennepin County residents who are experiencing homelessness can receive housing case management assistance through the Homeless to Housing program. We have solutions to homelessness, and have the tools to help people find and sustain housing.

Services

Homeless to Housing teams can help people staying in emergency shelter or living unsheltered with the following process as they work on a plan to get into stable and permanent housing: 

  1. Referral
  2. Intake
  3. Goal planning
  4. Barrier busting
  5. Housing identification and move in
  6. Stabilization and referral to long-term support
Eligibility

You are eligible for the Homeless to Housing program if you are:

  • Staying in a homeless shelter, or sleeping outside or in a vehicle
  • 18 or older
  • Not caring for children
  • Not already working with a housing case manager

Anyone in the homeless response system can add you to an interest list. That includes staff with: 

  • Outreach team
  • Streets to Housing
  • Homeless to Housing
  • Veterans Services
  • Hennepin County Shelter Team
  • Shelters
  • Drop-in centers
  • Adult Shelter Connect

What to expect from the process

Once you are added to the interest list, and as caseload openings become available, the team will follow up, confirm eligibility and then start working with you. If your contact information has changed recently, please ask any of the above advocates to update your contact information in HMIS because that is what the team will use to try and contact you.
