Omada Health online program
Omada Health is an easy-to-use online program for people at risk for type 2 diabetes or heart disease or living with diabetes or hypertension.
Omada is an innovative 16-lesson program that surrounds you with support to help reach your goals.
If you qualify, your personalized program includes:
- A a wireless smart scale to track your weight or a blood pressure monitor - depending on your program
- An Omada health coach to help keep you on track
- Weekly online lessons with healthy tips about better eating, fitness, sleep and stress management
- Support from an online group of people like you for added encouragement
Take the risk screening to see if Omada is right for you. (Use a web browser other than Internet Explorer to access this program.)
Participants who complete nine lessons of the program prior to September 1 will earn 1,000 wellness points. (Register for the program by June 1 to have enough time to complete nine lessons.)
Omada is available to adult Hennepin County Advantage and Standard health plan members who have risk factors for type 2 diabetes or heart disease or have been previously diagnosed with diabetes or hypertension.
Participants can continue with the Omada program after the initial 16 lessons as long as they are covered under the health plan. Participants learn more through a new curriculum of shorter weekly lessons, released every week for two years, that deliver smart, simple strategies for long-term healthy living; both the coach and the support of a small group are still available and the first 16 lessons are available for review anytime. Participants can keep their scale or blood pressure monitor, and can continue to use their web and mobile app.
