Health screenings
Onsite health screening events
- Health screenings will be offered at select Hennepin County locations by appointment. There will be limited or no availability for walk-ins.
- Only employees working onsite at these locations may participate in the screening event. There is no cost to the employee for the screening.
- Screenings include a fingerstick for glucose and cholesterol measurements, a blood pressure check, and optional height, weight, and waist measurements to calculate BMI.
- Participants will earn 500 wellness points. Points will be automatically uploaded. Please allow 4-6 weeks for the points to be reflected in your UMR Live Well account.
- It is recommended that the participant fast for 10-12 hours prior to their appointment.

- Create an account at My.QuestForHealth.com. (If you created an account for an onsite flu vaccination appointment in 2023, you can log in to your existing account.)
- The registration key is hennepincounty2024
- Your Uid is the first four letters of your first name, the first four letters of your last name, and the last four digits of your SSN (ex. janedoe1234, johnsmit1234).
- On your Quest dashboard under Wellness Screening, select Make an Appointment.
- Follow the prompts to select a location and make an appointment.
- Once you’ve scheduled an appointment, you’ll receive a confirmation email.

Other options for health screenings
Earn 500 wellness points for getting a health screening. A health screening includes measurements such as blood pressure, BMI, cholesterol, and glucose. Health screenings are covered at 100% at an in-network provider. Follow your provider's recommendations for receiving health screenings.
To earn wellness points, have your provider complete the Physican Lab Form and submit it to UMR. Print the form from your UMR Live Well account.

Individual health screening results are not shared with Hennepin County.
