Metropolitan Health Plan (MHP)

Metropolitan Health Plan is a nonprofit, state-certified health maintenance organization (HMO). MHP was first licensed by the state of Minnesota in 1983 and has been proud to offer exceptional care for our members ever since.

Cornerstone Solutions Medicare

Cornerstone Solutions is a Special Needs Basic Care (SNBC) plan for Hennepin County residents living with disabilities. To be eligible for enrollment, you must be certified disabled, between the ages of 18 and 64, and eligible for Medicaid and Medicare, or Medicaid only.

To enroll in Cornerstone Solutions, fill out this enrollment form (PDF) and mail it in, or contact Member Services for assistance. Enrollment specialists are available 8 a.m. to 8 p.m., seven days a week. Call 1-888-562-8000 (toll-free), 1-800-627-3529 (TDD) or 711.

Hennepin Health

Hennepin Health offers an integrated approach to health care by offering Hennepin County residents a plan that blends medical, behavioral health and social services. To be eligible for enrollment, you must be between the ages of 21 and 64, not have any dependent children, and be eligible for Medical Assistance (Medicaid).
