HSPHD is requesting proposals from qualified Group Residential Housing (GRH) programs. GRH is a State-funded income supplement program that pays for room-and-board costs for disabled or elderly, low-income adults in a qualified setting with which a county human service agency has a GRH agreement and has negotiated a monthly rate. The individual and the housing setting must qualify.

Hennepin County Human Service and Public Health Department (HSPHD) is seeking a qualified contractor to reduce exposure to secondhand smoke in multi-unit rental housing. The goal of this request for proposals (RFP) is to reduce tobacco-related morbidity and mortality by reducing exposure to secondhand smoke. The two primary targets of this strategy are public housing authorities and managers of privately-owned multi-unit rental housing in low-income areas, with an emphasis on publicly subsidized housing. The proposer will outline a plan of action to assist multi-unit housing managers implement smoke-free indoor air policies. The contractor will be required to participate in parts of a comprehensive evaluation of this strategy. However, the details of this evaluation may not be finalized until the time of the award.

Hennepin County Human Service and Public Health Department (HSPHD) is seeking qualified vendors to establish a Healthy Living Hub. The goal of this RFP is to expand the reach and impact of Healthy Living strategies to reduce obesity and tobacco use and to prevent or manage the chronic diseases that they cause. Hennepin County HSPHD seeks to identify agencies to establish and implement two Healthy Living Hubs for clearly defined populations. Selected agencies will leverage existing Hennepin County Public Health Promotion activities and create new opportunities for residents to increase healthy eating, active living, and smoke-free living within their communities.
A Parenting Assessment is a clinical assessment of a parent’s ability to function appropriately as a care-giver and to safely and effectively meet the individual needs of his or her children. It entails a culturally-sensitive review of a care-giver’s relevant parenting skills, knowledge, beliefs, attitudes, expectations, and behaviors, and identification of the parent’s specific areas of strength and concern related to parenting a particular child or children. The goal of the assessment is to determine the parent’s ability to provide a safe, healthy and nurturing environment for his or her children and to provide recommendations regarding how to effectively address any areas of concern identified.
Hennepin County HSPHD is seeking qualified agencies to provide Parenting Assessment Services for Child Protection clients.
The Department of Community Corrections and Rehabilitation – Juvenile Services Division is requesting proposals from agencies who are committed to working with correctional and HSPHD social service supervised youth and their families. Proposers must meet the required criteria detailed herein. Hennepin County wants to have the capacity to meet the cultural needs of the youth and families within the target population for sex offender services. Hennepin County is soliciting pre-applications for new permanent supportive housing projects to address housing and service priorities identified in the Heading Home Hennepin Plan to End Homelessness for persons who are homeless. Proposals for transitional housing are not being solicited.
Proposals for supportive housing projects that serve families, including but not limited to chronically homeless families are especially encouraged. Proposals that serve chronically homeless single adults, veterans, or youth are also encouraged.

For those who submitted proposals for the "Service Options for Persons Living in their Own Home", the process has been challenging and we are extending the selection process. Selections should be complete by the end of March and notifications will be provided at that time.
HSPHD is seeking responses from providers who are able to facilitate living arrangements for individuals in these populations that are consistent with the “community-living settings” definition in Minnesota Statutes 2011 Article 7, Sec. 41, Subd. 23. To be consistent with the statutory definition the living arrangements would not include Customized/ 24-Hour Customized Living or Supported Living Services in excess of 15-minute increments, and should include low concentration of people with disabilities.

Hennepin County HSPHD Case Management and Managed Care is requesting proposals from qualified vendors to provide Customized Living/24 Hour-Customized Living services for Hennepin County clients eligible for the Medicaid Elderly Waiver (EW). It is the County's intent to contract with multiple providers who serve eligible adult clients, especially those relocating from institutional settings. The services must be in Minneapolis or the Northern Suburbs and must include memory care.

Hennepin County Department of Community Corrections and Rehabilitation (DOCCR) – Juvenile Services Division is requesting proposals from qualified individual therapists to provide outpatient treatment for juveniles who have acted out sexually. These services are for male and female youth under court jurisdiction who are under age 21 and reside in Hennepin County.

The Hennepin County Community Corrections and Rehabilitation and the Human Services and Public Health Departments are requesting proposals from qualified individual psychologists to provide psychological evaluations (diagnostic assessment with psychological testing) for youth with open cases in juvenile probation and/or child welfare system which includes Child Protection, Mental Health Case Management, Initial Contact and Assessment, Out of Home Placement, and Transitional Youth Service Areas.
The Department of Community Corrections and Rehabilitation – Juvenile Services Division – Juvenile Detention Alternatives Initiative (JDAI) is requesting proposals from vendors to provide Community Coach services. The Community Coach Program is a community based alternative to detention. Youth who are released, or referred to a Community Coach, will receive assistance and support to ensure that they attend scheduled court dates, attend court ordered appointments, and do not commit additional offenses prior to scheduled court dates. Proposals are being solicited from qualified applicants that are committed to working with correctional youth. Applicants may be either agencies or individuals who meet the required criteria. The targeted service area is the Western suburbs of Hennepin County.
Department of Community Corrections and Rehabilitation (DOCCR) – Juvenile Probation Division is requesting proposals from qualified vendors to provide Evening Reporting Center (ERC) services for post adjudicated male youth under court jurisdiction who are ages 13 through 17 and reside in South Minneapolis and the cities of Richfield, St. Louis Park, and Bloomington.
Hennepin County is seeking proposals to contract with one or two providers to coordinate and oversee a program which utilizes the Multidimensional Treatment Foster Care model and implement two MTFC teams. One team will be certified to work children, ages 6-11, and the other team will be certified to work with older youth, ages 12-17.

The Hennepin County Department of Community Corrections and Rehabilitation – Juvenile Detention Alternatives Initiative is requesting proposals from qualified vendors to provide mentoring programs for our Somali youth and our Hispanic/Latino youth. These services are for post-adjudicated male and female youth under court jurisdiction who are ages 10 through 18 and reside in Hennepin County.
In October 2009, the Children’s Mental Health Collaborative (CMHC) approved a partnership with the Juvenile Detention Alternatives Initiative (JDAI) of Hennepin County to utilize a portion of the Local Collaborative Time Study (LCTS) funds. The funds will be divided equally between the two populations for culturally specific mentoring.

Hennepin County Human Services and Public Health (HSPHD) is soliciting proposals from qualified vendors to provide a comprehensive array of "After Hours" (11p.m. to 7 a.m.) crisis services. The target population is HSPHD clients and other Hennepin County residents and mandatory reporters, calling on behalf of clients, who are seeking to access Adult Protective, Adult Mental Health Crisis Services, Children's Protective, and/or Children's Mental Health Crisis Services.

Hennepin County is seeking proposals from vendors interested in contracting with Hennepin County to provide interpretation and translation services for the period of January 1, 2010 – December 31, 2013.
