Payment rates?
These maximum rates are set by the state.
Billing deadlines?
Providers must submit bills within 60 days of the last date of care or date billing form was sent, whichever is later. Payment will not be made on late bills unless provider shows good cause.

The Child Care Provider Guide (DHS 5260) will answer most of your general questions.

Need replacement billing form?
Contact the child care worker for a replacement billing form.
If parent does not pay co-pay?
Contact the child care worker. Benefits will end until the co-pay is paid or other payment arrangements are made with the provider.
